Privacy Policy

Prevention Assurance Inc. uses its company and Prevention Alliance websites to answer inquiries, receive Prevention Health Coach registration requests, and manage approved training access.

Information collected

The company contact form collects name, email, optional phone number, subject, and message. The registration form collects name, email, phone, street address, city, state, postal code, age, and optional notes. Staff accounts also use email, password credentials, role, permissions, audit events, and MFA setup status.

How information is used

We use contact information to answer inquiries. Registration information is used to review sign-up requests, avoid duplicate pending requests, communicate about the request, and provide approved course access. Admin account information is used to secure staff access and record accountability events.

Sharing

Approved learner information may be sent to Canvas or related course administration tools to create course access. Hosting, database, security, and email providers may process information for site operations. We do not sell personal information or share it for cross-context behavioral advertising.

Retention

Contact messages, pending and rejected applications, approved records, audit logs, and operational records are kept only as needed for communication, course administration, security, legal, and accountability purposes. Pending and rejected applications are also subject to configured retention controls.

Sensitive information

Do not submit medical records, diagnoses, payment-card details, passwords, or other sensitive information through the general contact form.

Security

The site uses HTTPS, limited staff permissions, audit logging, MFA support, CSRF protection, rate limiting, and database access controls. No internet service can be guaranteed completely secure.

Your privacy choices

Depending on where you live, you may request access, correction, deletion, restriction, portability, opt-out, or appeal review. We may need to verify your identity before acting on a request.

GDPR and international requests

If GDPR applies to your information, you may have rights to access, rectify, erase, restrict, port, or object to certain processing. We aim to respond within one calendar month unless an extension is permitted by law.

California privacy rights

California residents may request to know, delete, correct, and opt out of sale or sharing. We do not sell or share personal information as those terms are commonly used for advertising disclosures. We do not discriminate against users for exercising privacy rights.
